Meet virtually, one-on-one with your health coach twice a month to monitor and adjust your plan based on what you and your data tell us. All members get regularly scheduled visits and lab tests reviewed with your Evolve care team. If desired you can customize your plan at any time by adding on Power-Ups which may include additional visits with Coaches, Dietitians, Physiotherapists, Kinesiologists, group classes, additional continuous glucose monitors and more.
Health changes take time. We recommend a minimum of 3 months to work with our professionals to help facilitate positive, consistent behaviour change.
Prescribed CGMs are covered by some extended medical plans.
